There are two steps to sign-up for the HVJTMS golf events.

1. Complete and send in the Registration Form with payment.  If you are not a member of
one of the HVJTMS golf clubs, you may contact Lisa Reiher at reiher180@yahoo.com
or one of the following clubs which have agreed to allow non-members to represent them in the HVJTMS.
Stony Ford GC, Hickory Hill GC, Mansion Ridge,
and Turtle Creek @ Garden Cathay Resort.

2.  Complete and send with payment Coupon #1 and or Coupon #2
indicating which events you are going to play.
